As a progressive and vibrant healthcare organization, we are committed to being an innovative center of excellence in community hospital care.</p>Job Description<br><br><p>We are seeking a dynamic and experienced Clinical Resource Nurse to join our team in overseeing and coordinating community-based dialysis programs. The CRNs focus will be on growing our home dialysis options, and leading the transplant referral processes. The ideal candidate will have a strong background in nephrology, with the ability to transition into home dialysis modalities. In this role, the CRN will drive the expansion of home dialysis programs, and provide leadership in coordinating transplant referrals. The CRN will collaborate closely with multidisciplinary teams to enhance patient care and improve patient outcomes within the community.</p><ul><li>Take a leadership role in the coordination and expansion of community-based home dialysis programs, including peritoneal dialysis (PD) and home hemodialysis (HHD); focusing on increasing patient access and education.</li><li>Work closely with the care team to identify patients suitable for home dialysis, educating patients and families about treatment options, the benefits of home dialysis, and the home dialysis initiation process.</li><li>Oversee the logistical coordination of home dialysis.</li><li>Collaborate with home dialysis nurses and other clinical staff to ensure best practices and patient safety are maintained.</li><li>Lead the transplant referral process for eligible patients, ensuring timely and accurate referrals to transplant centers.</li><li>Work collaboratively with internal hospital stakeholders to continue to improve the transplant referral journey. </li><li>Assist with the strategic planning and implementation of expanding home dialysis programs into new communities and/or clinical sites.</li><li>Work alongside the management team to assess resource needs, staffing requirements, and logistical considerations for site expansion.</li><li>Evaluate patient feedback and program outcomes to ensure the home dialysis program is evolving to meet patient needs effectively.</li><li>Collaborate with nephrologists, home dialysis nurses, and other healthcare professionals to ensure integrated and patient-centered care across all modalities.</li><li>Participate in team meetings to review patient cases, identify potential challenges, and develop solutions that optimize patient care and treatment outcomes.</li><li>Participate in quality improvement initiatives aimed at improving patient satisfaction, safety, and clinical outcomes in both the home dialysis and transplant referral processes.</li><li>Guide patients and their families as they transition to their chosen modality, explaining the process and addressing any concerns or questions to promote understanding and engagement.</li></ul>Qualifications<br><br><ul><li>Current Registration with the College of Nurses of Ontario and BCLS Certification required</li><li>BScN as asset.</li><li>5 years <span >Nephrology/Dialysis</span> experience required</li><li>Demonstrated commitment to patient and family centered care</li><li>Previous nursing leadership experience</li><li>Excellent teaching, mentoring, interpersonal and public relations skills</li><li>Exceptional problem solving skills and critical thinking skills</li><li>Strong verbal and written communication skills</li><li>Able to work as a team member with peers, Program Leader, Patient Care Manager, Physicians and other multidisciplinary professionals</li><li>Demonstrated ability to support Program Leader, Patient Care Manager, Physicians and other multidisciplinary professionals and Halton Healthcare Services with managing change.</li><li>Contemporary thinking with positive progressive vision</li><li>Proficiency with Microsoft Word and Excel programs an asset</li><li>Satisfactory employment and attendance record</li><li>Must be able to meet the physical demands of the position.</li></ul><br><br>Additional Information<br><br><p><strong>Hours:</strong> Monday to Friday, days, 7.5 hour shifts, (Hours subject to change based on operational needs)</p><p><strong>Salary Scale</strong>: $44.55 to $61.95 per hour</p><p><strong>Date Posted:</strong> December 23, 2025</p><p><strong>Internal Application Due By: </strong> December 30, 2025</p><p><strong>REF3028T</strong></p><p></p><p>Halton Healthcare is an equal opportunity employer who is committed to integrating diversity, equity and inclusion throughout our operations, policies and culture.
