As a progressive and vibrant healthcare organization, we are committed to being an innovative center of excellence in community hospital care.</p>Job Description<br><br><p>The Registered Practical Nurse (RPN) will work in both the hospital and the community to support home dialysis patients. They establish caring, therapeutic relationships with patients receiving Home Hemodialysis and their families to attain or maintain optimal functioning by promoting health and/or treating health conditions through supportive, preventative, therapeutic, palliative and rehabilitative means. They will perform dialysis and associated intervention independently within their home. They will provide support to patients in the clinic setting as well as during home visits.</p><p>As part of the Independent Dialysis (home based dialysis) interdisciplinary team, the RPN will take a comprehensive approach to providing care for renal patients who are able to dialyze at home independently, either through  Home Hemodialysis. The role will include provision of training and ongoing follow up care and support. As part of the Nephrology interdisciplinary team, the RPN will practice within the scope of Nephrology nursing best practice, RPN scope of practice, nursing philosophy and the standards set by the renal program.</p><ul><li>Provides assessment, education, training and decision support for patients/family as it relates to independent dialysis</li><li><span >Coordinates/facilitates</span> appointments of patients including home visits throughout the community</li><li>Assessment of Dialysis patients utilizing  standards of care for Nephrology and  aligned with unit competencies</li><li>Pre/post dialysis machine management including preparation of circuit and programming and post dialysis disinfection</li><li>Initiation and discontinuation of dialysis treatment for stable patients</li><li>Monitoring patient and machine during dialysis therapy, providing intervention as necessary in collaboration with RN</li><li>Administer medications into the dialysis circuit as ordered by a physician and within scope of practice</li><li>Engages the interdisciplinary team in a collaborative approach to professional practice issues and acts in the role of Liaison between all members who participate in the care of the patient and their family</li><li>Provides a framework /smooth transition for patients who are involved in transitioning to home- based therapies</li></ul>Qualifications<br><br><ul><li>Current registration in good standing with the College of Nurses of Ontario required</li><li>Current BCLS certification required at time of hire.</li><li>Must be able to practice to full scope in this environment</li><li>Recent acute care experience required</li><li>Strong patient assessment skills required</li><li>Experience processing physician orders required</li><li>Sensitivity and knowledge of caring for the acute geriatric client is required</li><li>Ability to adapt quickly to new environments, remain flexible in change and collaborate within small teams is required</li><li>Good team player with positive attitude</li><li>Evidence of continued learning</li><li>Able to meet the physical demands of the position</li><li>Demonstrates being patient and consumer focused; makes a commitment to meet or exceed the needs of all internal and external customers</li><li>Strong communication and interpersonal skills; listens for clarity and meaning, and to speak in an honest and straightforward manner to ensure mutual understanding in a conversation</li><li>Demonstrates fiscal responsibility</li><li>Must have access to a vehicle for the purpose of transportation throughout the community</li><li>Valid Ontario drivers license required</li><li>Must be physically able to perform the essential duties of the position</li><li>Satisfactory employment and attendance record</li></ul><br><br>Additional Information<br><br><p><strong>Hours:</strong> Monday to Friday, Days; 0800 - 1600 hour shifts (hours subject to change based on operational needs)</p><p><strong>Salary:</strong> $35.29 to $38.16 per hour</p><p><strong>Date Posted:</strong> December 23, 2025</p><p><strong>Internal Applications Due By:</strong> December 30, 2025</p><p><strong>REF2993R</strong></p><p></p><p>Halton Healthcare is an equal opportunity employer who is committed to integrating diversity, equity and inclusion throughout our operations, policies and culture.
